Description

Direct Care Service Responsibilities:

 

  1. Review and assess referrals of individuals with an identified Substance Use/Opioid Use disorders (SUD/OUD) who are requesting treatment and/or Medication Assisted Treatment (MAT).
  2. Coordinate and provide direct services such as program assessments, SUD/OUD screens, intakes, counseling, DPH applications and developing reentry plans.
  3. Establish a supportive, motivational and therapeutic relationship with individuals within the parameters of a correctional environment.
  4. Oversee MAT Wellness and Treatment team
  5. Collaborate with the specialty courts, Parole and Probation to coordinate treatment planning for individuals in need of substance abuse treatment.
  6. Participate in multidisciplinary team \
  7. Coordinates with the SCSD medical and mental health team

Program Development:

  1. Assists in the implementation of treatment focused curricula for the purposes of group and individual counseling.
  2. Attends appropriate professional development trainings and presents at trainings for service providers and community groups as needed.
  3. Recruit and establish relationships with community providers for the purpose of providing additional treatment services to the incarcerated population at the SCSD.

Administrative:

  1. Assist the Assistant Superintendent in the implementation and oversight of the MAT Program
  2. Oversee and provide direct supervision to the Wellness Navigators  
  3. Keeps updated client records and class rosters for those participating in the MAT program.
  4. Prepare and maintain the necessary data collection required to provide to the Department of Public Health.

Requirements:

Must have a minimum of a bachelor’s degree and experience managing staff.  Applicant should have at least two years prior experience working with individuals with substance use disorders, providing supervision in a clinical or program setting as well as facilitating groups.  Correctional experience preferred, but not required.
